Evaluation and Treatment of Anxiety

There are several key components to successfully overcoming anxiety and panic disorder in my experience.

 

The first key to effective treatment includes developing a full understanding of your anxiety. 


While this sounds simple, it is often the missing piece of the puzzle.  Anxiety can stem from several different underlying problems.  Some examples include: a combination of recent events, previous life experience, childhood experience, genetics, lifestyle, personality traits, nutrition, substance use, supplements, and insomnia. All the relevant factors need to be identified and subsequently addressed over time.

After we identify all the contributing factors, we work together on a treatment plan that takes a step-by-step approach to eliminating your symptoms of anxiety while also addressing the underlying causes.  I have to get you feeling comfortable before starting to tackle some of the other pieces, so I start with making sure your symptoms are relieved.

 

Stabilization with medication is an option


 

Medication is a valuable tool that can offer relief of the more severe symptoms of anxiety.  It can often be used short-term while working on the underlying causes of your anxiety.  Some people genuinely need medication in the long-term, especially when there appears to be a strong family history of anxiety or a genetic component. In my experience however, I find that the vast majority do not require medication long-term, and the medication is weaned off very carefully as your anxiety resolves.

In terms of specific medication, I have found SSRI's to work well for anxiety and panic disorder. There seem to be a subset of patients who do not respond to SSRI’s.  In these people I find great success with an older class of medication called MAOI’s.  I find a trial of 2 SSRIs as an adequate first step and when someone does not find relief, I often quickly shift strategies and utilize an MAOI.  In more severe cases I combine more than one medication but typically try to avoid doing that if possible.

The next step in eliminating the anxiety is utilizing the appropriate modality of psychotherapy.


Finding the right type of therapy


Whether we use medicine or not, finding the right type of therapy is critical to the treatment plan. My initial goal is to get to know you as a person, make you feel comfortable and have you feel fully understood. With that foundation we can work together to figure out the right type of therapy modality may be the right fit for you. I work closely with different therapists who bring a unique skill set to the team. The goal is to get to the underlying emotions that are driving your symptoms. Combinations of different therapy approaches are often needed and I will make sure you are getting what you need as well as what you are comfortable with.


Underlying Medical Issues, Nutrition and Lifestyle

I have a passion for nutrition and exercise. I am a firm believer in small changes, and I will never overwhelm you or attempt to overall your entire life.  I will encourage you to attempt attainable goals with the goal of optimizing how you feel.  I believe in implementing some form of challenging exercise into your life. 

In my experience, I find re-creating struggle in the physical sense is more tangible than facing mental struggle. Becoming confident in overcoming physical struggle translates to confidence in making mental changes. This leads to momentum which is key to success.

I always look at any potential underlying medical causes such as insomnia, thyroid disease, stress, hormones and overall health.
